Background Check and Fingerprinting Form

Nursing Assistants work as caregivers in all types of health care facilities and agencies, but predominantly in long term care under the supervision of nurses. In order to be admitted into 91探花’s program, prospective students much complete the following Illinois Department of Public Health (IDPH) background check form and submit the Fingerprint Form below.

CNA Background Check and Fingerprinting Form

The CNA program requires applicants for the program to submit an Illinois Department of Health background check, as well as a fingerprint release form.
